I’m pleased to have the opportunity to work with you. This document outlines important information about my office policies and procedures. Please review it carefully and feel free to ask me any questions.

Appointments

  • Sessions are 53 minutes in length unless otherwise specified.

  • Appointments are scheduled based on mutual availability and are typically held either weekly or biweekly

  • Please arrive on time. Sessions cannot be extended due to late arrival.

Fees and Payments

  • The standard fee for individual therapy is $250 per session standard session. $300 for intake sessions

  • $225 per hour when scheduling EMDR blocks/intensives

  • Payment is due at the time of service for standard sessions and EMDR blocks.
    EMDR intensives are due 14 days in advance

  • Acceptable forms of payment include: Cash, Check, Credit/Debit Card, HSA/FSA Cards, Electronic Payment through TherapyNotes

  • Returned checks will incur a $30 fee.

Insurance

  • I am in-network with: Blue Cross Blue Shield, BCBS community solutions, Aetna, United Healthcare, Molina, and Meridian

  • It is your responsibility to verify your benefits and understand your insurance plan.

  • You are responsible for all fees not covered by your insurance company.

Cancellation and Missed Appointments

  • A minimum of 24 hours notice is required for cancellations.

  • Cancellations with less than 24 hours’ notice or no-shows will be charged $100 fee

  • Exceptions may be made for emergencies at my discretion.

Confidentiality

  • All information discussed in therapy is confidential and protected by law.

  • Exceptions to confidentiality include:

    • Threats of harm to self or others

    • Suspected abuse or neglect of a child, elder, or dependent adult

    • Court order or subpoena

    • As otherwise required by Illinois state law

Electronic Communication

  • Email and text messaging should be used for administrative matters only (e.g., scheduling).

  • Therapy sessions will not be conducted via text or email.

  • Telehealth sessions are available through TherapyNotes

Emergencies

  • I do not provide emergency services.

  • In an emergency, please call 911, or go to your nearest emergency room.

  • You may also contact the National Suicide & Crisis Lifeline by dialing 988.

Social Media Policy

  • I do not accept friend requests or interact with clients via personal social media accounts to maintain professional boundaries.

  • You are welcome to follow my professional pages for resources if available.

  • At times, I provide content for consumption to the public for educational purposes. You are not obligated to follow, like, or engage with this content. I will maintain confidentiality in this setting as well

Record Keeping

  • Your records are maintained in accordance with professional, legal, and ethical guidelines.

  • You may request to access your records with a written request.

  • Records are kept for a minimum of 7 years as required by Illinois law.

Termination of Services

  • Therapy may end when goals are met, if attendance is inconsistent, or if it is determined that another provider may better suit your needs.

  • You are free to discontinue therapy at any time.
